Ask DCAS to restore your standing on a list — for example, after a missed canvass.
This is a request — not an automatic restoration.
DCAS reviews each restoration request under the Civil Service Law and decides case by case. Submitting here routes your request to a DCAS supervisor; it does not change your list standing on its own.

Explain the circumstances. Don't attach medical records here — DCAS will request documentation securely if it's needed.
Guidance: most lists allow a limited number of restorations (commonly up to three). This is shown as guidance only — DCAS determines eligibility.
